    • Claim Form to report loss must be submitted within the same day of delivery
  • 7-Day Guarantee
    • Fish, Plants, Corals, and Invertebrates
    • Claim Form to report loss must be submitted within the 7-day guarantee
  • 30-Day Guarantee
    • Live Aquaria® Certified Captive Grown Corals
    • Claim Form to report loss must be submitted within the 30-day guarantee


Follow the steps outlined below to receive a store credit for the purchase price of an item if you experience a loss after completing the acclimation process and within the appropriate guarantee timeframe of your purchase

To expedite the issuing of store credit for your reported losses, please complete all of the required fields within the form. A digital photo of the deceased specimen must be attached to qualify for our guarantee(s). Please photograph the specimen on a white background, under adequate lighting, and outside of the aquarium or shipping bag.

Please Note: When submitting a claim form to report loss from a Pack, please submit a separate claim form for each unique loss within the Pack.
